Dengue fever is a mosquito-borne viral infection that is common in tropical and subtropical regions. It is caused by four closely related dengue virus types and is transmitted primarily through infected Aedes mosquitoes.

Many dengue infections are mild or produce no symptoms. When symptoms occur, they often resemble a severe flu-like illness, with high fever, headache, pain behind the eyes, muscle and joint pain, nausea, and rash. Most people recover, but a small proportion develop severe dengue, a potentially life-threatening complication involving plasma leakage, bleeding, low blood pressure, and organ dysfunction.

One of the most important features of dengue is that a person may begin to feel better just as severe disease is developing. For this reason, knowing the warning signs is essential.

What Happens During Dengue Infection?

After an infected mosquito bites a person, the dengue virus enters the bloodstream and begins multiplying. The body's immune response then produces the symptoms associated with infection.

The incubation period is commonly several days before symptoms appear. The incubation period for dengue fever is typically 4 to 10 days after a mosquito bite, with 4 to 7 days being the most common timeframe. The full range can span from 3 to 14 days before any symptoms appear. When illness develops, fever is often the first major symptom.

Dengue can range from an asymptomatic infection to severe disease. The severity cannot always be predicted from the initial symptoms, which is why monitoring is particularly important during and shortly after the fever begins to subside.

Common Symptoms of Dengue

Typical dengue symptoms may include:

    • High fever
    • Severe headache
    • Pain behind the eyes
    • Muscle and joint pain
    • Nausea or vomiting
    • Skin rash
    • Weakness and fatigue
    • Easy bruising or small areas of bleeding under the skin

The combination of intense muscle and joint discomfort has contributed to dengue's reputation as a particularly painful viral infection.

Symptoms may initially resemble other mosquito-borne or viral illnesses. This can make laboratory testing important when dengue is circulating in the community or when someone has recently traveled to an area where dengue is common.

When Does Dengue Become Severe?

The most dangerous stage can occur as the fever starts to improve.

Severe dengue develops when the infection causes significant changes in blood vessels and circulation. Increased vascular permeability can allow plasma to leak from the bloodstream into surrounding tissues. At the same time, platelet levels can fall and abnormal bleeding may occur.

This combination can result in dangerously low blood pressure, shock, internal bleeding, and organ damage. Managing blood pressure in dengue might be different to the BP management, but requires immediate recognition of plasma leakage and dehydration, which cause blood pressure to drop.

Warning Signs of Severe Dengue

Seek emergency medical care immediately if a person with suspected or confirmed dengue develops:

    • Severe or persistent abdominal pain
    • Repeated or severe vomiting
    • Vomiting blood
    • Blood in the stool
    • Bleeding from the nose or gums
    • Extreme weakness or unusual tiredness
    • Restlessness or irritability
    • Other sudden signs of deterioration

These symptoms can progress rapidly, so waiting for them to disappear at home can be dangerous.

How Does Dengue Spread?

Dengue is primarily transmitted by infected Aedes mosquitoes. A mosquito becomes infected after biting a person carrying dengue virus and can subsequently transmit the virus when it bites another person.

This transmission cycle explains why dengue can spread rapidly in areas where suitable mosquito populations are present.

Dengue is not normally spread directly from person to person through ordinary contact such as touching, coughing, or sharing food. However, infection can have implications during pregnancy because transmission from a pregnant person to the baby can occur.

Dengue and Its Relationship to Other Diseases

Dengue belongs to a broader group of mosquito-borne viral diseases. Several infections can occur in the same geographic regions because they are transmitted by similar Aedes mosquito species.

Dengue and Zika

Zika virus and dengue virus are different viruses, but they have an important epidemiological connection: both can be transmitted by Aedes mosquitoes.

Their symptoms can overlap. Dengue commonly produces high fever, significant muscle and joint pain, headache, and sometimes bleeding, while Zika is often associated with a milder illness characterized by rash, fever, joint pain, and conjunctivitis.

The distinction is especially important during pregnancy because Zika infection can cause serious complications for a developing fetus.

Because dengue and Zika can circulate in the same areas and produce overlapping symptoms, laboratory testing may sometimes be necessary to determine which infection is responsible.

Dengue and Chikungunya

Chikungunya is another mosquito-borne viral disease transmitted by Aedes mosquitoes. It can produce fever and prominent joint pain, making it easy to confuse with dengue based on symptoms alone.

However, the illnesses are caused by different viruses and can have different complications and treatment considerations.

The shared mosquito vector means that prevention measures—particularly reducing mosquito exposure and eliminating standing water—can help reduce the risk of several infections simultaneously.

Who Is More at Risk?

Anyone living in or traveling to an area where dengue transmission occurs can become infected.

Transmission risk is influenced by exposure to infected mosquitoes and local transmission levels. Severe illness can be more concerning in certain groups, including young children, older adults, pregnant individuals, and people with certain underlying health conditions.

A particularly important factor is previous dengue infection. Because there are four dengue virus types, infection with one type does not provide complete protection against infection with the others. A subsequent infection with a different type can sometimes be associated with a greater risk of severe disease.

How Is Dengue Diagnosed?

Dengue is diagnosed using a combination of the person's symptoms, travel or exposure history, and laboratory testing.

Blood tests can look for evidence of the dengue virus or the body's immune response to infection. Testing can also help doctors distinguish dengue from other infections that produce similar symptoms.

This distinction matters because dengue can require different management from illnesses such as Zika, chikungunya, malaria, or other febrile infections.

Doctors may also monitor blood counts and other laboratory measurements during illness. A falling platelet count, for example, can be an important finding, although platelet levels must be interpreted together with the person's overall condition.

How Is Dengue Treated?

There is no routine specific antiviral treatment that eliminates dengue infection. Management primarily focuses on hydration, rest, symptom control, and careful monitoring for complications.

People with uncomplicated illness may be advised to drink adequate fluids and rest.

For fever and pain, acetaminophen/paracetamol may be recommended when appropriate. Acetaminophen is safe for dengue fever and pain, but overdose causes severe liver toxicity.

However, people with suspected dengue should generally avoid aspirin and nonsteroidal anti-inflammatory drugs such as ibuprofen unless specifically advised by a healthcare professional, because these medicines can increase bleeding risk.

Severe dengue requires hospital treatment. Depending on the patient's condition, treatment may involve intravenous fluids, careful monitoring of circulation and organ function, and blood products when significant bleeding occurs.

How Long Does Dengue Last?

Dengue symptoms commonly last 2-7 days, and many people begin recovering after the acute febrile phase.

However, improvement in fever does not necessarily mean the danger has passed. The period around fever resolution can be when severe dengue becomes apparent.

Fatigue and weakness may persist after the acute illness has resolved, so recovery can take longer than the initial fever period.

Can You Get Dengue More Than Once?

There are four dengue virus types, and infection with one type generally provides lasting immunity to that particular type but not complete protection against the others.

This means a person who has previously had dengue can become infected again.

Importantly, a previous infection does not mean every subsequent infection will be severe, but repeat infection with a different dengue virus type can be associated with increased risk of severe disease.

How Can Dengue Be Prevented?

Because mosquitoes transmit dengue, preventing mosquito bites is one of the most effective protective measures.

Helpful prevention strategies include:

    • Use an appropriate insect repellent.
    • Wear clothing that covers exposed skin.
    • Use screens on windows and doors.
    • Use mosquito nets where appropriate.
    • Remove containers that collect standing water.
    • Empty buckets, tires, pots, and other water-holding objects regularly.
    • Keep outdoor areas free of unnecessary stagnant water.
    • Follow local public-health recommendations during outbreaks.

These measures can also reduce exposure to Zika and chikungunya, since the same Aedes mosquito group can transmit all three viruses.

When Should You Seek Medical Care?

Anyone who develops a high fever and other dengue-like symptoms after living in or traveling to an area where dengue occurs should consider contacting a healthcare professional.

Emergency care is particularly important when severe abdominal pain, persistent vomiting, bleeding, extreme weakness, restlessness, or sudden deterioration develops.

The timing is important: severe dengue can emerge when the initial fever is improving, so feeling temporarily better does not always mean the illness is over.

There is no simple cure for uncomplicated dengue, so early recognition, appropriate hydration, careful symptom management, and monitoring are central to treatment. Most people recover, but recognizing severe dengue early can be lifesaving.
