Acute Liver Failure: Causes, Warning Signs, Diagnosis, and Treatment

Acute liver failure is a rare but potentially life-threatening condition in which the liver suddenly loses its ability to perform essential functions, often within days or weeks. Unlike chronic liver failure, which develops gradually after years of liver damage, acute liver failure can occur in someone with no known history of liver disease.

The liver processes nutrients and medicines, helps remove harmful substances from the blood, regulates metabolism, and produces proteins needed for blood clotting. When severe liver-cell injury occurs suddenly, these functions can deteriorate rapidly. This can lead to abnormal bleeding, kidney problems, infection, and swelling of the brain.

Also called fulminant hepatic failure, acute liver failure is a medical emergency. Some cases can be reversed when the cause is treated quickly, while others may ultimately require a liver transplant.

How Is Acute Liver Failure Different From Chronic Liver Disease?

The key difference is the speed of deterioration.

Chronic liver disease develops over a long period and may eventually lead to cirrhosis and chronic liver failure. Acute liver failure, in contrast, involves severe liver injury developing rapidly, followed by impaired blood clotting and neurological changes known as hepatic encephalopathy.

Clinically, acute liver failure is associated with severe acute liver injury, impaired coagulation, and newly developing encephalopathy in a person without preexisting cirrhosis. This distinction is important because someone can appear relatively healthy shortly before becoming seriously ill.

What Causes Acute Liver Failure?

Acute liver failure might cause by several factors. One of the most important causes is acetaminophen toxicity. Acetaminophen, also known as paracetamol, is found in many prescription and over-the-counter medicines. Taking too much can overwhelm the liver's normal metabolic pathways and produce a toxic metabolite.

Normally, the liver neutralizes this metabolite using glutathione antioxidant. During an overdose, glutathione can become depleted, allowing the toxic compound to damage liver cells.

Acetaminophen-related liver failure can follow a single large overdose or repeated excessive use over several days. Importantly, treatment should not wait for severe symptoms. N-acetylcysteine (NAC) is the established antidote and is most effective when given early.

Viral Hepatitis

Viral infections are another important cause. Hepatitis A, hepatitis B, and hepatitis E can occasionally produce severe acute liver injury. Other viruses, including herpes simplex virus, cytomegalovirus, and Epstein-Barr virus, can cause severe liver injury in particular circumstances.

Most viral hepatitis infections do not progress to acute liver failure, but severe cases can deteriorate rapidly. Identifying the specific virus therefore helps guide treatment and prognosis.

Medicines, Supplements, and Toxins

Certain prescription medicines, including some antibiotics, anticonvulsants, and nonsteroidal anti-inflammatory drugs, can cause severe drug-induced liver injury in susceptible people. Herbal medicines and dietary supplements may also contribute.

Other causes include autoimmune hepatitis, Wilson disease, obstruction of hepatic blood flow, and severe reduction of blood flow to the liver during shock or critical illness. Rare causes include acute fatty liver of pregnancy, heat stroke, certain cancers, and poisonous mushroom exposure.

In some cases, even extensive investigation does not identify a definite cause.

Early Warning Signs of Acute Liver Failure

The first symptoms may be relatively nonspecific. They can include:

      • Nausea or vomiting
      • Loss of appetite
      • Weakness or malaise
      • Pain or tenderness in the upper-right abdomen
      • Yellowing of the skin or eyes, known as jaundice
      • Abdominal swelling
      • Increasing sleepiness
      • Confusion or disorientation

Jaundice occurs when bilirubin accumulates in the bloodstream. As liver function deteriorates, neurological symptoms can become increasingly important.

Why Confusion Can Be a Dangerous Sign

Changes in mental function may indicate hepatic encephalopathy, a serious complication of acute liver failure.

A person may initially become unusually sleepy or have difficulty concentrating. This can progress to confusion, disorientation, personality or behavioral changes, severe drowsiness, and eventually coma.

One contributing factor is the accumulation of substances such as ammonia when the failing liver can no longer process them effectively.

Severe acute liver failure can also cause swelling of the brain called cerebral edema. Increased pressure inside the skull can result in neurological deterioration and seizures, making sudden mental-status changes particularly concerning.

Why Liver Failure Can Cause Bleeding

The liver produces many proteins required for normal blood clotting. When liver function deteriorates, production of these proteins decreases, resulting in abnormal coagulation and an elevated INR (International Normalized Ratio).

This can contribute to bleeding, including gastrointestinal bleeding. However, an abnormal INR does not automatically mean that a person will experience bleeding, so doctors interpret coagulation results together with the overall clinical condition.

This is an important distinction: laboratory abnormalities can indicate serious liver dysfunction, but they must be interpreted in context rather than considered individually.

Kidney Problems and Other Complications

Acute liver failure can affect organs beyond the liver.

Kidney injury may occur, particularly in severe acetaminophen toxicity or advanced systemic illness. Severe liver dysfunction can also disturb circulation, metabolism, immune function, and other physiological processes.

Patients may become more vulnerable to infection, especially during critical illness and hospitalization. In severe cases, liver, kidney, circulatory, neurological, and immune problems can develop together, making rapid medical management essential.

How Doctors Diagnose Acute Liver Failure

Diagnosis involves identifying the combination of acute liver injury, impaired coagulation, and altered mental status.

Blood tests commonly evaluate:

      • Liver enzymes and bilirubin
      • Blood-clotting measurements
      • Blood glucose
      • Kidney function
      • Electrolytes
      • Other indicators of systemic illness

Doctors also investigate the underlying cause through testing for viral hepatitis, autoimmune disease, acetaminophen exposure, medications, toxins, and metabolic disorders.

A detailed medication and supplement history is particularly important. Imaging may be used to examine liver structure and blood flow. In selected cases, a liver biopsy may be considered, sometimes using a transjugular approach when coagulation abnormalities make conventional biopsy more hazardous.

How Is Acute Liver Failure Treated?

Treatment usually requires hospitalization and may require intensive-care monitoring, particularly when neurological or other organ complications develop.

The first priority is to identify and treat the underlying cause.

For acetaminophen toxicity, N-acetylcysteine is the key treatment because it helps restore glutathione-related defenses and reduce toxic liver injury. It is most effective when started early. NAC may also be used in some cases of non-acetaminophen acute liver failure while the cause is being investigated.

Other treatments focus on complications. Doctors may closely monitor brain function, blood glucose, kidney function, electrolytes, circulation, and infection. Severe brain swelling requires specialized neurological management.

When Is a Liver Transplant Needed?

The liver has a remarkable ability to regenerate, so some people recover if the underlying injury is reversible and sufficient functional liver tissue remains.

However, continued deterioration despite intensive treatment can make liver transplantation necessary. Patients with acute liver failure may therefore be treated or transferred to specialized centers capable of performing transplantation.

The decision depends on the cause of liver failure, neurological condition, coagulation abnormalities, kidney function, metabolic problems, and the overall progression of the illness.

Importantly, not everyone with acute liver failure needs a transplant. Some patients recover completely when the cause is identified and treated promptly.

Can Acute Liver Failure Be Prevented?

Some causes are preventable.

Acetaminophen should always be taken according to recommended instructions, and combination medicines should be checked carefully to avoid accidentally taking acetaminophen from multiple products.

People should also tell healthcare professionals about all prescription medicines, over-the-counter products, herbal preparations, and supplements they use.

Vaccination and infection-prevention measures can reduce the risk of certain viral hepatitis infections, while avoiding unknown poisonous mushrooms and unsafe substances can prevent some toxic exposures.

However, conditions such as autoimmune disease, inherited metabolic disorders, vascular problems, and some infections cannot always be prevented.

When Should You Seek Emergency Care?

Because acute liver failure can progress rapidly, sudden warning signs should never be ignored.

Immediate medical evaluation is particularly important if someone develops yellowing of the skin or eyes, upper-right abdominal pain, unusual sleepiness, confusion, personality or behavioral changes, or other unexplained neurological symptoms.

A suspected acetaminophen overdose also requires urgent medical attention, even if the person initially feels well. Early treatment can reduce the risk of severe liver injury.

The most important principle is speed. Rapid recognition, identification of the cause, appropriate treatment, intensive monitoring, and timely evaluation at a transplant-capable center can make a critical difference. Some patients recover as the liver regenerates, while others require transplantation.

Sudden jaundice accompanied by abdominal symptoms or changes in mental state should therefore be treated as an emergency, not something to monitor at home.
